What is double eyelid surgery?
Double eyelid surgery or Asian blepharoplasty is a surgery to permanently create upper eyelid creases. Double eyelid surgery is associated with East Asian people, as 50 percent of East Asian people are born without their supratarsal or upper eyelid crease. In medical terms, one who had a “monolid” or an obfuscated crease is due to the different points of fusion between the orbital septum and the levator aponeurosis.
For one with a normal creased eyelid, the levator aponeurosis is fused with the orbital septum approximately 8-10 mm above the supra-tarsal border, leaving a fusion of the aponeurosis, septum, and tarsal plate, which creates a fold in the skin at least 3-4 mm above the lid margin. However, people who lack a superior eyelid crease had the levator aponeurosis insert lower than 8mm above the supra-tarsal border, resulting in a fusion of the aponeurosis, septum, and tarsal plate at less than 2 mm above the lid margin, too short to create a fold visible as the eyelid crease.
In addition, 90 percent of Asian adults are characterized by what is called the Epicanthal Fold – a crescent-shaped portion of skin at the medial aspect of the superior lid that descends along the border of the nose toward the canthus to meet the medial portion of the lower eyelid. In combination with a lack of eyelid crease, the fold also leads to a greater intercanthal distance, resulting in visibly smaller eyes compared to normal ones.

How is double eyelid surgery done?
Same as the standard eyelid surgery, the double eyelid surgery is considered a minor surgery for those who wish to have a larger, more rounded eye appearance. While the results should be a permanent double eyelid crease, a surgery may end with eyelids with uneven height or shape, excessively high folds, blepharoptosis, ectropion, or upturned cilia. However, these complications are preventable if the procedure is done under professional hands.
The ideal candidate for double eyelid surgery should be a non-smoker, healthy individual aged over 20 that does not have medical conditions that can impair healing or serious eye conditions. We recommend that you consult with an ophthalmologist for an appropriate treatment plan if you are currently with eye disease like glaucoma, dry eyes, or detached retina; diabetes; thyroid disorders; and cardiovascular disease, including high blood pressure.
If you are a fit candidate for a double eyelid surgery, after a proper eye and facial assessment by trained physicians, including a vision test and photography. The current common technique for double eyelid surgery includes a non-incisional suture ligation technique and an external incisional surgical technique. For the non-incisional suture ligation technique, a crease is created by suture fixating the orbicularis oculi muscle or the subcutaneous tissues to the levator aponeurosis or tarsal plate. Meanwhile, the external incisional surgical technique uses an incision on the upper eyelid 7 mm from the ciliary margin. This was closed by passing a suture through the lid from the conjunctiva to the superior tarsal border of the anterior skin edge to the tarsal plate.
The double eyelid surgery would last only a few hours. If your condition is stable after the operation, you can leave for home with a return for the removal of the surgical stitches after 4-6 days. For the post-surgical care, most of them are no different from normal eyelid surgery, with a possibility of blurred vision, watering eyes, swelling, bruising, and some discomfort. This could be relieved by cool compresses to reduce swelling with ice packs, sleep with your head raised higher than your chest, prescribed eye drops or ointments, and paracetamol for pain management. Bruising may be visible for two weeks. A desk job is allowed after a week, while driving has to wait for 2 weeks, and physical activity for at least 2-4 weeks.
