What is a labiaplasty?

Labiaplasty is an aesthetic surgical procedure for the reconstruction of the labia majora or outer labia and labia minora or inner labia, which are both skin folds of the vulva. Speaking in easier terms, labiaplasty is a process of “trimming and wedging” the tissue of the labia to improve the size, shape, appearance, and comfort. These procedures had to be done by certified plastic surgeons, as with other cosmetic and functional plastic surgeries.

While natural labia come in an array of shapes and size, and may change following multiple circumstances – including hormone changes, piercing, childbirth, and aging, a common preferable form including the labia minora that do not extend beyond the labia majora, particularly when standing; the clitoral hood that reasonably short and non-protuberant, without excess folds;  the labia majora that be full but free of redundant skin or excessive fatty tissue that could cause an unsightly bulge under clothing; and also the mons pubis should exhibit mild fullness without protruding through clothing.

Why do you need a labiaplasty?

As mentioned earlier, labiaplasty could be a process that helps improve physical, cosmetic, and emotional reasons. Speaking in detail, labiaplasty is a rational choice for the operation for three reasons.

  • Reduction of the size of the labia minora : the excessive tissue of the enlarged labia minora could be twisted, turned, pinched, or tugged. These could lead to discomfort and irritation during sports and physical activities; redness and pain caused by tight clothes; dyspareunia or tissue being pulled into the vaginal canal during intercourse, resulting in pain; and an increased risk of urinary tract infections (UTIs)
  • Reduction of an asymmetry of the labia majora and labia minora : an asymmetry not only affects confidence, but also comfort and sexual well-being. In addition, some may prefer a neater or more “tucked-in” look of the size or length of their labia.  
  • Complete reconstruction of the labia : there are occasions that the labia may need a complete rebuild surgery, including congenital defects, post-traumatic treatment, post-cancer resection, and operation for gender-affirming surgery.

What do you need to know before considering labiaplasty?

While labiaplasty is eligible for a woman between 18 and 50, earlier attempts are not recommended due to possible changes in the conditions that could affect your labia, unless she requires reconstructive surgery due to congenital defects or traumatic conditions. Here, labiaplasty is one of the recommended solutions for women aged 30 or more who have a situation amenable to surgical correction. 

Nevertheless, labiaplasty contained some risks as with other plastic surgeries – bleeding, bruising, infection, wound breakdown, and scarring. For a specific risk of labiaplasty, one of the possible cases is ongoing pain, pain with sex, or loss of sensitivity. In addition, inadequate or excessive trimming of your labia tissue may occur and require additional surgery. Thus, those could be prevented with adequate evaluation, appropriate planning, and precision surgery.

Here, a fit candidate should be healthy, a non-smoker, not menstruating, and not under an active gynecological disease, including malignancy and infection. If you are considered a fit one, other than normal preoperative examination – including blood sampling and urinalysis, the visual examination with photography in the lithotomy position is required for the alignment between the patient’s expectation and the surgeon’s suggested plan. In addition, a psychological evaluation is needed to ensure that the patient has realistic expectations and is pursuing the procedure for appropriate reasons.

If the decision is made, you may be asked for a temporary halt of food and drink, adjustment of your medicines, and what to wear to and from the procedure. Also, you may be asked to shave your pubic area. On the operation day, after the cleansing and disinfection of the operational area, IV sedation and anesthesia are applied. The urinary catheter may be deployed in case of an autonomous urinal during the procedure

The labiaplasty itself then could be done via a trim technique that involves trimming excess tissue with a scalpel or scissors, followed by suturing of the cut, or a wedge technique by resecting only a “wedge-” or “V”-shaped portion from the most protuberant area of the labia minora. Meanwhile, reconstructive surgery uses nearby tissue for the reconstruction of the labia. Most labiaplasty operations take a few hours to complete, except for a complete reconstruction that takes a longer time. After the suture is completed, you will be monitored for bleeding and any other complications. If everything goes right, you will be discharged on that day with follow-up appointments, necessary medication, and instructions to properly clean the area to reduce the risk of infection. 

For the after-effects and post-operation care. Swelling, discomfort, and pain may be possible, but could be dealt with over-the-counter pain relievers. Bleeding may last for a week, which sanitary pads could help absorb. Light work is possible after a few days, but exercise, physical activities, and sexual intimacy have to wait between 4-6 weeks after the surgery. Final results may take up to 4 to 6 months to show up with little or no scarring.
